**MONT. ESTHETICS, LLC. – BOOKING POLICY** By booking an appointment with MONT. ESTHETICS, LLC., you acknowledge and agree to the following policies. These policies ensure fairness, protect appointment availability, and maintain a smooth scheduling process. 1. BOOKING AGREEMENT     •    A credit card on file is required for all bookings. Your card will not be charged at the time of booking unless specified below.     •    If booking online, no deposit is required.     •    If booking directly (via text, DM, etc.), a non-refundable deposit of 50% of the service cost is required via CashApp ($MONTESTHETICS) within 24 hours of booking or the appointment will be canceled.     •    The remaining balance must be paid via debit/credit, cash, or CashApp on the day of service. 2. COMPLETED BOOKING FORM     •    All clients must complete one consent form for each appointment type scheduled.     •    If there have been changes in skincare products, medical conditions, or medications, a new form must be submitted before the appointment.     •    The completed form must be submitted within 24 hours of booking or the appointment will be canceled.     •    MONT. ESTHETICS, LLC. is not responsible for any adverse reactions, damages, or complications resulting from the client’s failure to disclose updated medical or skincare information on the form. 3. CANCELLATION & RESCHEDULING POLICY     •    Cancellations/reschedules must be made at least 12 hours before the appointment.     •    Cancellations within less than 12 hours of the scheduled time will result in a charge of 75% of the service cost to the card on file.     •    One-time rescheduling is allowed past the grace period, within 7 days of the original appointment, to avoid the cancellation fee. 4. LATE ARRIVALS POLICY     •    A 10-minute grace period is provided for all appointments.     •    If you arrive after 10 minutes, your appointment may be canceled at the artist’s discretion.     •    If no communication is received within the grace period, it will be considered a no-show and charged accordingly.     •    No extra time will be added to your appointment if you are late within the grace period. 5. NO-SHOW POLICY     •    A no-show results in a charge of 100% of the service cost to the card on file.     •    No-call, no-shows will be banned from future bookings unless a new 50% deposit is paid. 6. PAYMENT & CREDIT CARD AUTHORIZATION     •    By booking, you authorize MONT. ESTHETICS, LLC. to charge the card on file for any applicable fees outlined in this policy.     •    All payments are final except as noted in the cancellation and rescheduling policies.     •    Chargebacks and payment disputes will result in a permanent ban from future appointments. 7. SERVICE POLICIES     •    No makeup: Please arrive with a clean face to ensure the best results and maintain the scheduled time.     •    No caffeine: Avoid caffeine on the day of your appointment to minimize eye movement during the service.     •    Health disclosure: By booking, you confirm that you are not sick and that any medical conditions or medications affecting lash retention have been disclosed to your technician.     •    Aftercare instructions: Will be provided after service and must be followed to maintain retention and lash health. 8. GUEST POLICY     •    Guests are allowed but must remain in the waiting area during the service. 9. LEGAL AGREEMENT By booking an appointment with MONT. ESTHETICS, LLC., you acknowledge and agree to all policies above. You understand that failure to comply may result in additional charges or restrictions on future bookings. MONT.
